  • Medical History for concurrent chronic disease undergoing treatment
  • Laboratory Results
  • Diagnostic Images and Reports: Radiographs, Ultrasound, CT or MRI images (when applicable)
  • Up To Date Rabies Certificate
  • Not required for pets under 6 months of age or pets who have been diagnosed with cancer/tumors. For cancer/tumor patients, we do encourage the most recent rabies certificate to be included with medical records, even if it has expired.
  • Patients who are not up to date on rabies vaccination or have not received a rabies vaccination may be refused a consultation.
