Client Appointment Requests
A referral from your pet’s primary care veterinarian is required to schedule a consultation. You may request an appointment and one of our team members will contact you (typically within 48 “business” hours) to follow-up with your appointment request.
Medical records are required in order to ensure our services are appropriate for your pet’s needs.
- Medical History pertaining to problem requiring referral
- Medical History for concurrent chronic disease undergoing treatment
- Laboratory Results
- Diagnostic Images and Reports: Radiographs, Ultrasound, CT or MRI images (when applicable)
- Up To Date Rabies Certificate
- Not required for pets under 6 months of age or pets who have been diagnosed with cancer/tumors. For cancer/tumor patients, we do encourage the most recent rabies certificate to be included with medical records, even if it has expired.
- Patients who are not up to date on rabies vaccination or have not received a rabies vaccination may be refused a consultation.

Consent forms for diagnostic and surgical procedures are available by request and can be emailed directly to the client.
